Group Homes
SHC owns and operates nine Medicaid funded homes throughout Medina County. In these family-type homes, individuals receive 24-hour care to meet their adaptive living, personal care, community access, therapeutic and behavior support needs in a highly-regulated, licensed setting. Shared Living Settings & In-Home Supports
SHC provides homemaker/personal care, community access and support services to individuals in a home or apartment with up to four individuals sharing the home. Typically, these settings include the availability of staff 24-hours a day, seven days a week. As a certified provider of waiver and supported living services our agency provides homemaker/personal care, community access and support services to individuals who reside in their own apartment, home, or with their family. These services can be designed to meet the unique needs of the individuals based on assessment and funding. Respite Services
Respite services include those services provided in a residential or recreational setting on a short-term basis, for the primary purpose of giving the primary caretaker a break from the responsibility of routine care. These services may be provided in the individual’s home, a group setting, or at an alternative site. Ann Tubbs Estates (affliate program)
Ann Tubbs Estates is a 40-unit apartment complex located in Medina that provides housing for individuals with disabilities. This complex is HUD-subsidized and provides independent housing for adults who are eligible through this government program. Basic supports and community activities are included.
